New Patients
Initial Visit
When scheduling a new patient appointment, your first visit will be to establish care during which you may be able to address only a few medical issues. If you are acutely ill, or have more than 1-2 issues to discuss, you may need to make another appointment so the provider can adequately address your needs.
Prior to Your Initial Visit
For your initial visit, we will need a completed New Patient Forms from you which includes registration, consents, and health history.

The New Patient Forms for adults, pediatric under 13 and weight loss only forms are linked below, but you will also get an e-mail with the forms attached.

Please bring your completed New Patient Forms with you to your first visit. This allows more time for you to visit with the provider.
If you are not able to complete the New Patient Forms prior to your appointment, please arrive 30 minutes before your scheduled appointment to complete the forms. Your appointment time will not be extended to accommodate time needed for you to fill out your New Patient Forms.
